Harmony Family Health Clinic is a Nurse Practitioner-led clinic that provides primary health care services to Albertans of all ages.

We are currently accepting new primary care patients. Please note you must be a resident of Alberta to access these services. We do not offer walk-in services.

Fees for Uninsured Services

The following services are not covered under Alberta Health. The following fees will apply. Payment is required before services are rendered. Only debit and credit cards are accepted (no cash).

Driver's Medical (<75): $150
Driver's Medical (75+): $100
Employment Medicals: $120
Insurance Medicals: $120

Work/Sick Notes: $40
Modified Duties Notes/Forms: $50
Professional Opinion Letter: Starting at $100 depending on complexity
Copy of Chart Notes for Insurance/Lawyer: Starting at $50
Long Term Disability Form/Letter: Starting at $100 depending on complexity

Medication Authorization Insurance Forms: $50
Disability Insurance Forms: $120
Dental/Private Pre-Op Forms: $120
Disabled Parking Placard: $25

Veterams Affairs - Basic: $100
Veterans Affairs - Complex: $150 (Lumbar/Sacral Spine, Cervical Spine, Gastrointestinal, Neurology)

Missed Appointment: $50

Please note that NPs do not have any control over any rejected insurance claims, and appeal forms will be subject to a $50 fee.
