Clinic Policies
​No Show/No Answer/Late Arrival Policy
24 hour notice is required for any appointment cancellation (if your appointment is on a Monday you must phone before 9:00 am). A late arrival is considered to be 10 minutes past your scheduled appointment time (or 5 minutes for a phone appointment). A fee of $50 may be charged, and this fee must be paid for before your next appointment.
Late Arrivals
Any patient who is more than 10 minutes late to their scheduled appointment time (without notificaiton) will forfeit their appointment and will need to re-book. Please call the office if you are running late (please note we cannot guarentee that you will still be able to be seen as a late arrival affects other appointments).
Phone Appointments
All phone calls from the NP will be from a blocked number - please ensure your phone is able to receive calls from masked numbers. The NP will only phone one number one time - please ensure you confirm your preferred phone number with reception when booking your appointment. If you miss a call, please phone reception at 780-418-5212 within 5 minutes of the missed call, otherwise it will result in a missed appointment. You will receive your call within 20 minutes of your scheduled appointment time - if you have not received a call after 20 minutes please phone reception at 780-418-5212.
Walk-In Availability
Walk-ins are subject to practitioner availability. It is recommended to phone ahead and book a same-day appointment if available.
Uninsured Services
Uninsured services, such as work notes, insurance forms, and driver's/employment medicals, are subject to fees. Only debit and credit card may be used to pay for these fees. Fees are due at the time of the appointment before services are rendered. Please note that a completed insurance form does not guarantee that it will be accepted by the insurance company; refunds will not be issued if coverage is denied.
Forms Completion Timeline
Forms will be completed and submitted/returned to the patient within 10 business days.
Patient Behaviour
Harmony Family Health Clinic does not tolerate any rude, threatening, or violent behaviour from patients or visitors. If a clinic member feels you or your support person are violating this policy you will be asked to leave and may be denied future services. If you have pre-paid for a form/medical/note you will not be refunded.
Washrooms
Please use the public washrooms in the hallway outside of the clinic door. The clinic washroom is reserved for staff and urine sample collections.
Accessibility
There is an elevator available in the main entrance.
Shoe Policy
We kindly ask that all patients remove their footware in the clinic, in all weather and all seasons. Disposable foot coverings are available upon request if you are not wearing socks.
Scent Policy
We kindly ask for you to not wear any scents (perfume, cologne, body spray, etc.) during your appointment.
Mask Policy
Any patient over 2 years of age experiencing signs or symptoms of respiratory illness (fever, cough, runny nose) will be required to wear a mask while in the clinic; if you do not wish to wear a mask you may request a phone appointment instead. Masks are available to any patient or visitor who would like to wear one.
Photos, Videos, and Recording:
You may not take any photos, videos, or record any conversations with any staff member or NP without their express consent. Any approved audio recordings must begin with a statement spoken by the NP and is not to be erased. Recordings are not to be edited. Failure to comply with this policy will result in being de-paneled and you will no longer be able to attend any medical appointments at Harmony Family Health Clinic.
We understand that Canada has a one-party consent law for recording, however, this is a clinic policy that will be enforced.
